video uploading file error

After click upload a .mp4 or .flv file of about 5MB size, 100% progress, approved promt, then apear the error message "uploading file error" in a popup with black backgroud.

I'm using dolphin 6.1.6, running on winxp, apache 2.2.

More Info.

I tried MUSIC module to  upload two file with .wma and .mp3 , and both uploaded successfully. But only the .mp3 file can be played.

 When play the .wma file, the player just seems running with a pause button appear, but no sound out, and the progress button don't move. 

I wonder what formats the player support.

Problem solved, thanks all ! Just replace the ffmpeg.exe with a latest version.
