unusual UNDEFINED tags on music and video

I've run into a bit of problem that I can't seem to solve. I've gone thru the appropriate files but can't find anything that would refer to this.

The MUSIC & VIDEO UPLOADS are where the errors are occuring. When trying to upload either I get the standard upload form but the boxes (title,tags,description) all read undefined. The small icon links also read undefined.

I am inserting a pic to show this..... ANY help would be most appreciated.

Actually I went thru the entire section and double checked....  MY server, so lots of access..  php is ok... settings are still in place, replaced "ray" dir completely and checked htaccess.

It almost looks like a database error.. but they are there and working..  when I try to find the "command" that would retreive those specific "items" and place them..... it just gives me a ray script.....

I've messed a little with styling and page layout but at this point I can't narrow down which files they may have been.

I'm now comparing an original archive list with my published server list to compare dates.... see which ones have been altered....   but this is becoming a very tedious task.

Any one ???
