Look at your url string.

compared to:

/home/a9225164/public_html/inc/header.inc.php

go to inc/header.inc.php and check the path on line 135 that it's trying to call.

This is a perfect example of why you don't bury dolphin in a subdirectory and then edit it with multiple usernames.
