Is there a survey module available for purchase?

or

A Polls module that allows for more than one choice?

I checked the market place and did not see anything that would meet the above requirements.

I am looking for something where a person can choose one or more items.. Primarily, my goal is to find out where visitors and members are finding my advertising in the print media...

so, they could check one or more items and then click submit.

Let me know when you have something ready for me to take a look at...

You can have unlimited answers.

Newton, I think the poll only allows for one selection?

Newton, by one choice selection, I mean of a several choices to "vote" on, you can only select one option.

Am I wrong in this? I have not looked at this module since day one... :)
