Polls

I am looking for a mod to Dolphin 6.1.1 existing polling capabilities, or an add-on if one exists.

- Poll title, questions, and multiple answer formats (radio, checkbox, text box)
- Ability for users to create polls. Admins should also be able to create polls on behalf of the site or certain users.
- Track users who have responded (their user id and response).
- Users may only vote once per poll.

Does anyone know if there is any poll mods out there that have these features or can recommend someone to do the work? 

Current poll system at 70% are fit your requirements (it without track system)

and now this is under development (we renew this mod by your wishes  ;)
