Dependent Fields Not Working

I followed installation  steps which is there in given link http://www.boonex.com/m/Dependent_Fields, but Its not working. Could you please help us and do installation of Dependent field on our boonex server.

hey Leonids I Don't have vendor direct contact number and reference. please send us Vendor detail .I think AQB is vendor but i don't have there contact details

I posted on dependent field module forum but I didn't get any reply, I had also given username and password of boonex for installation to them but I can't see any changes in dependent field module.

Yes, you've given the access to your server, but we've replied to you yesterday via email, didn't you receive our email? The module is not installed on your server. So what changes to the module do you expect to see if the module is not installed? Please install the module and provide a step by step instruction on how to reproduce the problem - only in that case we can look into the problem and fix it (if it is exist).
