Database error in Dolphin: cannot create new user profiles!

Hi Guys

First time posting, although I've been lurking on the forum for the last couple of months as I develop my site. So far I have always been able to locate a solution to any issue, but not this time! :-(  I need your help!

A couple of days ago, I was doing some to tweaks to the member profile in Dolphin and must have modded, removed or moved something which I shouldn't have because since then, I can no longer create any more new user profiles! At the point of clicking the join now button on the join.php page, the website throws up a database error message on the next page.

This does not affect my ability to login front end with a user profile I already had and also does not affect login to the back-end in the admin panel.

I am attaching a doc with the e-mail error message I have received (it's pretty long), but I think the most interesting bit is probably this:

Mysql error: Duplicate entry '' for key 2

Found error in the file '/home/bywize/public_html/inc/classes/BxDolProfilesController.php' at line 116.

Called 'db_res' function with erroneous argument #0.

This may be unrelated, but as part of my mods, I had removed the following field from the profile:

_FieldCaption_EmailNotify_Join:

It did not look like a front end item and did not seem to be doing anything there, but I am not sure if removing it has caused the problem. I have tried to restore this field to see if this would fix the problem, but it has not (and interestingly it now appears as a tick box rather than a text box like it did previously...).
