Site errors (not in database)

I've started to get an error with the "member login" for my site.  This is the second time I've received this error and after the last time I just re-made a template and the issue was solved.  I'm not sure how this error was caused or how to even fix it since I dont know where its even set.  I've checked the member.php file and still dont see anything wrong,  I've replaced the member.php file and it still doesnt change it.  I've included 2 screenshots for your viewing pleasure.  (you may have to right-click and "view image")

On the "home page/index" the Username, and Password field do NOT display.  If you select "My Account" it takes you to the member.php script which shows the  __or__ __join_label__ and then the user/pass fields.

Any ideas?

Hi,
I'm a little confused - not unusual ;). If the login box does not display, does that mean you are unable to login? ... if so, how is the 'my account' link visible? If you have used the top (log in or join now) link to login, its maybe more confusing as to why it should be asking for a password when you are already logged in..
Can you provide a link to the page rather than just a pic?

To the issue of the login not showing on the homepage:
Does the homepage builder (admin panel) still show see more 'login' as included?
If it is still there and for some reason it is not being called i would imagine its not the php file, but potential a change in the css. Have you altered any of the css files recently? Might explain that or/join/label if its an open parameter.

I've temporarly replaced the css directory and the index.php to the factory one and still the same issue.

The issue is that the Login form is NOT showing up for members who are NOT logged in. Meaning if you go to my site it does NOT show Login [ ] / Password [ ].

This is an issue in the template i'm sure as switching to the default template it displays properly. I have done a LOT of editing of scripts but when I finished editing things were still working. I came home from work and see more noticed things were not working properly. If it isnt in the scripts/index.php or in the css/ what else could it be?
Okay I solved the issue, Aparently my server is somehow reverting permissions on directories and files. I'm not sure as to what is causing this problem but it really is starting to be an annoying issue. [Solved/Closed]

Wondering who your server is, as I've encountered the same trouble! I have the hostforweb which was dolphin recommended and now wondering about them!!!
I host my own server on a dedicated dual T1 at my office :)
