Hooking Up Gong Script

I have a card reading program that is not hooking up with my mysql. I can not go back to vendor as he does not sell these programs anymore. Any takers? I know its a simple job but I am not familiar with mysql issues. I know it's not fetching the answer to the card cause of one line.

Oh, "card reading program" usually calls a software for smartcards reading such as bank (credit) cards, but you have some playing cards php/js script. Was confused by description, sorry.
