Contest Mod Needed

Need a Contest Mod that has the following:

All contests-- user will either upload a Video or Choose to Write an entry. 

Member selects contest to join, then selects either Video Entry or Writing Entry, then checks off  "agrees to terms and conditions" box. User then submits there entry. The submitted entries will only go to admin and not be public. If admin wants, admin can post entries on site for anyone to vote on.  The voting must limit the person voting to only one vote (even if they are not a member or signed in) (either daily or other).
