Quiz Module

Hello, I am looking for a quiz module that is capable of attaching multiple pictures, videos and sounds with user defined correct answers.

I have been looking in the market for some time and haven't found anything yet.  I would also like to track the statistics of the answers and display them in the activity feeds.
